Join Our Compassionate Team as a Hospice Registered Nurse Case Manager!

Salary Range: $76,000 – $79,000 DOE

Serving the Southern Region:
Sikeston • Charleston • Benton • East Prairie • Portageville

As a Hospice RN Case Manager, you’ll do more than provide care — you’ll bring peace, comfort, and dignity to patients and families during life’s most sacred journey.
Your expertise, empathy, and heart will make a profound impact every single day.

What You’ll Do

Deliver skilled, compassionate nursing care in patients’ homes and facilities.
Conduct comprehensive assessments and develop individualized care plans that honor patient wishes.
Provide education, emotional support, and guidance to families and caregivers.
Coordinate care with the interdisciplinary team to ensure seamless communication and holistic support.
Administer medications, treatments, and symptom management with excellence and grace.
Participate in on-call rotations to ensure patients always receive timely, dignified care.

What You Bring

Active RN license (required).
Experience in hospice, palliative, geriatric, or home health care (preferred).
A calm presence, strong clinical judgment, and a compassionate heart.
Ability to work independently while collaborating with a dedicated care team.
Excellent communication, documentation, and organizational skills.
A genuine calling to serve, comfort, and advocate for quality of life.
