Salary Range: $84,000 – $92,000 DOE

What You’ll Do

• Conduct timely, comprehensive admission assessments for new hospice patients in their homes, facilities, or hospitals.

• Educate patients and families about hospice philosophy, services, and the plan of care — ensuring understanding and emotional comfort.

• Collaborate with physicians, intake, and interdisciplinary team members to ensure seamless, well-coordinated transitions to hospice.

• Complete documentation and orders accurately, efficiently, and in compliance with regulatory standards.

• Provide emotional support, compassionate communication, and reassurance throughout the admission process.

• Represent Seasons Hospice with professionalism, empathy, and warmth — every single time.

What We’re Looking For

• Current RN license in the State of Oklahoma.

• Hospice, palliative, or home health experience preferred (but not required).

• Strong assessment, communication, and teaching skills.

• Excellent organizational abilities and confidence working independently in the field.

• A calm, compassionate presence that brings comfort to patients and confidence to families
