Join Our Compassionate Team as a Hospice Registered Nurse Case Manager

Salary Range: $84,000 – $86,000 DOE

As a Hospice RN Case Manager, you’ll do more than provide care — you’ll bring peace, comfort, and dignity to patients and their families during life’s most sacred journey. Your expertise, empathy, and heart will make a meaningful difference every single day.

What You’ll Do

Deliver skilled, compassionate nursing care in patients’ homes and facilities.
Conduct comprehensive assessments and develop individualized care plans that honor the patient’s wishes.
Provide education, guidance, and emotional support to families and caregivers.
Coordinate care with the interdisciplinary team to ensure seamless communication and holistic support.
Administer medications, treatments, and symptom management with excellence and grace.
Participate in on-call rotations, ensuring patients always receive the care they deserve.

What You Bring

Active RN license (required).
Hospice, palliative, geriatric, or home health experience preferred.
A calm presence, strong clinical judgment, and a compassionate heart.
Ability to work independently while staying connected to a supportive team.
Exceptional communication, documentation, and organizational skills.
A genuine calling to serve — to listen, comfort, and advocate for quality of life.
