We’re seeking a dedicated and compassionate RN Case Manager to join our growing healthcare team. In this vital role, you’ll serve as a clinical leader and patient advocate—coordinating care, guiding families, and ensuring seamless, compassionate support across homes, nursing facilities, and assisted living communities.
What You’ll Do
As an RN Case Manager, you’ll play a central role in shaping the patient experience by:
- Conducting comprehensive patient assessments and developing individualized, patient-centered care plans.
- Collaborating closely with an interdisciplinary care team to ensure seamless communication and coordination.
- Managing discharge planning and transitions of care, ensuring patients and families feel supported every step of the way.
- Monitoring patient progress and adapting care plans as needs evolve.
- Educating and empowering patients and caregivers with clear guidance, compassion, and advocacy.
- Maintaining accurate, timely documentation in compliance with healthcare regulations.
- Staying current on best practices in care management to continuously improve patient outcomes.
On-Call Expectations
- Participation in backup on-call rotation is required as part of this role.
- Backup on-call supports patient needs after hours, weekends, or holidays as scheduled.
- On-call responsibilities are shared and planned in advance to support work-life balance.
What We’re Looking For
- Current Registered Nurse (RN) license in the state of practice.
- Strong background in patient assessment and case management.
- Experience in geriatrics, long-term care, home health, or hospice is highly valued.
- Knowledge of discharge planning and managed care principles preferred.
- Excellent communication, organization, and time-management skills.
